Waiver of Liability and Hold Harmless Agreement *

• I acknowledge (or parent/guardian) that I am aware that training and practice in Brazilian Jiu-Jitsu, Muay Thai Kickboxing, and/or MMA is physical, tough, and competitive. I understand there is strength and conditioning training, self-defense training, grappling, and full-body contact involved. Physical injuries are possible. In consideration of being allowed to participate I assume voluntarily the inherent risks including but not limited to: bumps, bruises, strains, scratches, soreness, cramps, and even the possibility of more serious injuries such as sprains, broken bones, permanent disability, or even death.
• I accept (or parent/guardian) full responsibility for the use of, or participation in, any and all classes, competitions, services, equipment, demonstrations, or events, owned, operated, or sponsored by SSBJJA LLC Carlson Gracie Team, whether on-site or off-site for my negligence or negligence of others.
• I (or parent/guardian) hereby release and agree to indemnify and hold harmless SSBJJA LLC Carlson Gracie Team, its landlords, its owners, officers, directors, members, employees, volunteers, representatives, and/or agents from any and all loss, claims, injury, damage, or liability sustained, or incurred by me or my family. This release shall be binding upon the estate, heirs, next of kin, executor and/or administrator, or any entity making claims against SBJJA LLC Carlson Gracie Team on behalf of me or my family.
• I (or parent/guardian) grant the right to use and publish SSBJJA LLC related photographs of me or in which I may be included at SSBJJA LLC facilities or competition locations.
